Ultrasonographic Findings of Bile Duct Stones in Different Intrahepatic Location

Abstract
Accurate localization of intrahepatic duct stones is essential to good prognosis and selection of adequate therapeutic methods.
Stones localized in one segment of intrahepatic biliary tree were countered as one case, and ultrasonographic findings of these stones were classified into three types; type I is intraphepatic shadow echo within dilated duct, type 11 intrahepatic shadow echo with proximal duct dilation, type III intrahepatic shadow echo without dilated duct. One hundred and fifteen cases of 44 patients, consisted 10 men and 34 women, were studied by sonography using 3.5 and 5 MHz transducers, and all studies were performed by one radiologist. Ultrasonographic findings of intrahepatic duct stones were analyzed and correlated with intrahepatic location and presence of common bile duct stone.
Twenty cases were type 1, 35 cases type Il, and 60 cases type Ill. Type Ill was the most frequent type in all intraheaptic locations, and was located in left lobe (30/67) and in right lobe (31/48). There were more common bile duct stones with type I intraheaptic stone than with other types. In 63 cases with accompanying common bile duct stone, only three cases (4.7%) were located in left hepatic duct (LIh), and in 52 cases without common duct stone, 22 cases (42%) in left hepatic duct.
